Lower Cervical Spinal Dural Arteriovenous Fistula with Extradural Hematoma in a 12-year-old Boy Presenting with Alternating Hemiplegia

The majority of spinal dural arteriovenous fistulas (SDAVFs) develop in elderly patients and are located between the lower region of the thoracic spine and the upper region of the lumbar spine. Herein, we report the case of a 12-year-old boy diagnosed with lower cervical SDAVF and a concurrent extradural hematoma. The patient complained of weakness in his upper and lower extremities with alternating hemiplegia as an initial symptom. A spinal magnetic resonance imaging (MRI) scan revealed an extradural mass lesion with a heterogeneous signal intensity at the C7- T2 level. The mass had low signal intensity at the T2 level. The timely use of an MRI scan facilitated a prompt diagnosis. The patient¡¯s symptoms drastically improved following a decompression laminectomy. The pathologic findings of the specimen were consistent with arteriovenous fistula. We report this case because it is clinically important due to the rarity of lower cervical SDAVF in pediatric patients. Furthermore, early diagnosis and treatment are required to prevent irreversible neurological deficit.
